Glucagon-like peptide 1 receptor agonists (GLP-1RAs) have emerged as breakthrough agents for weight loss. However, discontinuation is common, and clinical trials have demonstrated significant weight regain following cessation. A systematic review carried out by a team of students at Trinity College and School of Clinical Medicine at the University of Cambridge (Brajan Budini and Steven Luo, co first authors), has investigated the impact of stopping the medication, modelling the trajectory of weight regain over 12 months and beyond.
Their findings are published in eClinicalMedicine with Professor Antonio Vidal-Puig from the IMS-MRL as a senior co-author along with Dr Adrian Park, the lead obesity consultant at Cambridge University Hospitals NHS Foundation Trust (Addenbrooke's).
The review represents the most comprehensive collection of evidence on post-GLP-1RA weight outcomes to date and possibly the first systematic review to parametrically model the trajectory of weight regain over time after discontinuation of GLP-1RA therapy. The researchers identified a regain curve in which 60% of the weight lost during treatment is regained 1 year after cessation. Extrapolation of their model provides novel insights into weight outcomes beyond 1 year, predicting that weight regain plateaus at 75% of the weight lost during treatment.
Given the growing reliance on GLP-1RAs for weight management and the high discontinuation rates reported in previous studies, the authors suggest that physicians and regulatory bodies must utilise alternative dosing strategies to help mitigate the risk of weight regain. Current evidence suggests that adopting an individualised dose-tapering approach can limit weight regain, but more research is needed to develop strategies for reliable long-term weight management after GLP-1RA cessation.
